A Holistic Approach to Community-Based Care

Assertive Community Treatment (ACT) is a team-based approach to delivering comprehensive, flexible treatment and support to individuals with the most severe and persistent mental health challenges.

ACT offers individuals struggling with mental illness an intensive, community-based mental health program model that brings together a team of professionals to take a holistic approach to treatment, factoring into treatment everything from medications to finances, housing and other everyday challenges.

Extensive studies have shown that ACT programs can greatly reduce the likelihood of costly psychiatric hospitalizations and provide clearly defined, result-based support for individuals struggling with mental health disorders.

In 2008, Greater Nashua Mental Health Center was selected by the New Hampshire Bureau of Behavioral Health as the pilot site for the implementation of ACT.
